Insurance Plans

We participate with many insurance plans and will gladly process your claim. Please bring the necessary information with you on your first visit.

For your convenience, we accept payments made with cash, check, Visa, MasterCard and American Express. Financing is also available through Care Credit.

As a dental provider, our relationship is with you and not your insurance company. Insurance policies generally cover only a portion of the total treatment cost. If your insurance company does not cover the estimated charges, the balance becomes your responsibility. Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment.

*Effective July 1st, 2016, our office will not be able to accept new patients with insurance plans following the Dentemax fee schedule, however we will continue to provide services for existing Dentemax patients.*
